Track how specific reframes landed across sessions — what worked, what didn't, and what adjustments improved them.
This worksheet helps therapists track the effectiveness of specific reframes over time — documenting the immediate client response, whether anything shifted in subsequent sessions, and what would be adjusted on a second attempt. The pattern across entries is often more informative than any single entry.
Complete this worksheet after every session in which a reframe was offered. After four to five entries, review them together before the next session in which a reframe is planned. The log builds a picture of how this specific client responds to reframes and when they land.
