Body Scan Log for Unexplained Physical Anxiety Symptoms
Helps clients identify patterns in physical anxiety manifestations that are not tied to specific.
Sometimes a client’s chief complaint is the physical experience of anxiety itself. They describe a racing heart or shortness of breath that seems to appear from nowhere, detached from any particular worry or rumination. This disconnect makes the anxiety feel random and particularly difficult to address, leaving both of you without a clear entry point for intervention.
This log offers a structured approach for the client to observe these physical events in their full context. Instead of relying on memory, the client builds a concrete record of the circumstances surrounding each incident. The result is a much clearer picture of the patterns at play, providing a more reliable foundation for your work together.
Body Scan Log for Unexplained Physical Anxiety Symptoms
For the next seven days, use this log to record physical sensations that arise without a clear thought attached. When you notice a symptom like a racing heart, tightness in your chest, or tingling, pause and make an entry. Note the date and time. Describe the specific sensation and where you feel it in your body. Record what you were doing or what was happening around you just before you noticed it. Do not search for a worrying thought to explain the feeling. The focus is purely on the physical event and its context. Finally, rate the intensity of the sensation on a scale from 1 to 10, where 1 is barely noticeable and 10 is very strong.
| Date/Time | Sensation & Location | Activity/Situation | Intensity (1-10) |
